Description of problem:

Upstream linux have reduced some duplicate codes in #VC code path, unified some guest terminate codes in #VC code path, and improved instruction decode in #VC code paths.

In addition, upstream Linux also fixed some bugs, such as unexpected nested #VC caused by wrongly DR7 accesses.
